Stonewall Staff Feedback Questionnaire 2012

Thank you for volunteering to fill in this short questionnaire. We are interested in your experiences in the workplace as a lesbian, gay or bisexual person. The questions should take no longer than five minutes to complete.
The information you provide is anonymous, completely confidential and will go to Stonewall's Workplace Team, not to your employer. Once the data is collected and analysed, only your organisation's average scores will be shared with your employer and individual responses will not be able to be identified. Summary data from this survey will be presented in a variety of formats, including a report.
Please note that specific cases of harassment or bullying will not be fed back to your employer. If you wish to report a case to your employer, you should follow the established reporting procedures within your organisation.
